Sample Single Page Disclosure The FCRA requires a single page disclosure and executed authorization.
Sample Single Page Disclosure with Credit The FCRA requires a single page disclosure and executed authorization. This sample incorporates language when you are including a credit report as part of the consumer or background report.
Sample Adverse Action Letters Employers and others as defined by the FCRA must send various notices to the subjects or consumers of background checks prior to taking adverse action, such as a decision to not hire or terminate employment. These notices, often referred to as Pre-Adverse Action and Adverse Action, must be transmitted by SecurTest clients to the subject or consumers, unless the client has contract such with SecurTest or iReviewNow, LLC.
